Anatomy of the Breasts

Breast surgery is a form of surgery performed on the breast. There are multiple types of breast surgeries, depending on the patient need,medical history and existing health conditions. Some are performed for medical reasons such as cancer, others for cosmetic reasons.
Anatomy of Breast
Breasts, also known as mammary glands, are a pair of glandular organs that produce milk in response to the hormone changes of childbirth. They are mainly made up of fatty tissue and are supported by ligaments and large muscles.

Each breast has 15-20 lobes with a number of lobules and ducts supportive tissue (see diagram). Each lobule has about 30 major ducts that open onto the nipple. The darker area of skin around the nipple is called the areola. At the edge of the areola there are large glands that produce fluid to lubricate the nipple.

In each armpit, there are about 20-30 lymph nodes (glands) that drain fluid from the breast. These form part of the lymphatic system that helps the body to fight infection.

Breast  - Benign

A benign breast condition refers to a lump, cyst, or nipple discharge (fluid) of the female or male breast that is not cancerous. Other common conditions are breast calcification,  mastitis & hyperplasia.

  1. Breast Pain 
    • Cyclical  - Due to Hormone Stimulation
    • Non - Cyclical - Due to Infection (mastitis), Mondor's disease
  2. Nipple Discharge
    • Milk during breast feeding
    • Bloody / Clear / Purulent from duct disorders
  3. Breast Lump - Solitary discreet
    •  < 30 yrs  - most likely Fibroadenoma
    • 30 - 50 yrs - Fibroadenoma / Fibrocystic disease
    • 50 yrs - most likely malignancy
    • Diffusely bumpy - Fibrocystic disease

Mammography
When the cells in the breast tissue grow and divide out of control, it creates a mass of tissue called a tumor. When the tumor is caused due to abnormal cells it is called malignant (cancerous). These abnormal cells can spread to other parts of the body infecting them as well. 

Breast cancer is the most common type of cancer affecting women in India. If detected early, breast cancer can be treated well with minimal morbidity to the patient.

  • A lump or thickening in or near the breast or in the underarm
  • A change in the size, shape, or contour of the breast.
  • A blood-stained or clear fluid discharge from the nipple.
  • A change in the skin on the breast or nipple (dimpled, puckered, scaly, or inflamed).
  • Redness of the skin on the breast or nipple.
  • A stone-like hardened area under the skin.

Breast lumps are usually suspected or felt during self-breast examination or clinical breast exam. If you have symptoms or lump, you may have to undergo one or more of these follow-up tests to rule out or confirm the presence of cancer:

  • Diagnostic Mammography
  • Ultrasound
  • MRI
  • Biopsy

Your treatment plan could involve a combination of surgery, radiation therapy, chemotheraphy, hormone & Immunotherapy etc.

A diagnostic mammogram is an X-ray examination of the breasts. Mammograms uses low-dose x-rays to discover unusual signs or symptoms like lumps or abnormalities in one or both the breasts more closely. This is commonly used during an initial breast cancer screening.

Mammography can sometimes cause slight discomfort for a very brief period of time as it involves compression of the breast. Ideally, mammograms are scheduled a week after woman’s menstrual cycle so that the breasts are less tender.

Below is the recommendation of American Cancer Society for breast cancer screening for women.

  • Women between 40 and 44 have the option to start screening with a mammogram every year.
  • Women 45 to 54 should get mammograms every year.
  • Women 55 and older can switch to a mammogram every other year, or they can choose to continue yearly mammograms. Screening should continue as long as a woman is in good health and is expected to live at least 10 more years.

My mother was diagnosed with cellulitis and her hand was swollen and she experienced a lot of pain. Upon recommendation from our general physician, we consulted Dr. Sanjitha at Apollo Jayanagar. We had a great experience with her. At the very beginning she made my mother comfortable in the way she interacted with her. Following a very thorough examination she recommended certain exercises and medications for my mother which helped her greatly. Her swelling decreased in a week’s time and the pain has also come down significantly. Most importantly there was no surgery required. Her reassurance during this entire phase and the confidence she gave to all of us was noteworthy. Highly recommend Dr Sanjitha.

Dr. Sanjitha perfomed my surgery and treatment was done with utmost care. She explained the issue in detail to us and made us aware of the options available for treating the same. I was diagnosed with abscess with pain and feverish symptoms. She completed the surgery very professionally and took great care to ensure that there was very minor/minimal scar arising out of surgery. She personally completed every dressing session post surgery which was for about a month and very closely monitored the progress of my health and healing of the wound. I was very comfortable to approach her with my queries and seek help. I thank her for her efforts and helping me recover fast.
